The correct answer is A. It is quite coincidental that I happened to be moderating the ASN CPC and a case of adenovirus nephropathy was also presented! There is a beautiful discussion of this topic in JASN, as well a description of a case in AJKD in 2003. It is also discussed on the Nephron Power blog. The biopsy often shows a granulomatous interstitial nephritis, tubular necrosis, and ground glass-like intranuclear viral inclusion bodies in tubular cells caused by an adenovirus (ADV) infection.
Brief Case Presentation: The patient's biopsy was sent to the Brigham by a pathology colleague from Colombia for special staining. A 50 year-old male status post-kidney transplantation suddenly develops septic shock with rapid renal dysfunction (serum creatinine is 6 mg/dL).
