iSEDIMENT

The Question
This is an iSEDIMENT pic from taken by Nikolaus Koegl, an Austrian medical student who rotated with me on the renal service this past week.  The arrow points to a cast. What is it?

The Answer
In my opinion this is a mixed cellular cast with both renal tubular epithelial and white blood cells in the cast.
